Transcript & Letter Request Form

Use the Transcript & Letter Request Form to request a letter of good standing, enrollment verification, transcript, or rank verification.

All requests for transcripts must be made in writing to the Office of the Registrar. Requests must include the student's full name, student ID, graduation date (if applicable), number of copies needed, the address where transcripts should be sent, and the student's original signature. There is a $2.00 charge per copy payable by check, cash or money order. Please send requests to the appropriate campus.

All requests for letters of good standing, enrollment verification or rank verification must be made in writing to the Office of the Registrar. Requests must include the student's full name, student ID, the name of the person/organization and address to which the letter should be addressed, and the student's original signature.
